Tumor Necrosis Factor-alpha (TNF-ɑ) is a cytokine important in nearly all acute inflammatory reactions by acting as an amplifier of inflammation. TNF-ɑ blockade has been used worldwide to treat inflammatory conditions, e.g., rheumatoid arthritis. Targeting TNF-ɑ to treat cancer and cardiovascular diseases is also being evaluated in clinical trials.
TNF-ɑ belongs to the TNF superfamily. Proteins of this family can be released from the cell membrane by extracellular proteolytic cleavage and then function as cytokines. TNF-ɑ trimer exerts function by binding to the trimeric receptor TNFR-1 (CD120a) or TNFR-2 (CD120b). FAS, CD40, CD27, and RANK are also members of the TNF receptor superfamily. Interestingly, TNFR1 signaling is pro-inflammatory and apoptotic. It activates NF-kB, mitogen-activated protein kinases, and death signaling. The production of other pro-inflammatory cytokines (IL-1, IL-6, GM-CSF, etc.) may be induced subsequently. In contrast, TNFR2 signaling tends to be anti-inflammatory and promotes cell proliferation. It activates the canonical NF-kB signaling and PI3K/Akt pathways.
